Set up a proper culture of mentoring and training in your org to turn people who are a little too green or inexperienced into effective team members.
Not as catchy as "fire fast", but infinitely more sustainable.

My guess would be that most "bad hires" are either insufficiently skilled or insufficient motivated. For the former, you could provide training if you at least see potential. The latter shouldn't be too difficult to figure out.
